The Assessment of SpondyloArthritis international Society (ASAS) classification criteria were developed to identify patients with axSpA (including nr-axSpA and ankylosing spondylitis [AS]) (Figure 3). In patients with chronic lower back pain for ≥3 months, with age of onset <45 years, classification of axSpA depends upon satisfying the requirements of either an imaging arm or a clinical arm of the classification criteria.1 The imaging arm requires evidence of sacroiliitis by magnetic resonance imaging (MRI) or radiograph* plus ≥1 SpA feature, such as inflammatory back pain (IBP), uveitis, dactylitis, or a family history of SpA. The clinical arm requires human leukocyte antigen B27 (HLA-B27) positivity and ≥2 SpA features.1 IBP is a key feature of axSpA. Key features of IBP include age at onset less than 40 years, insidious onset, improvement with exercise, no improvement with rest, and pain at night with improvement upon getting up.1 Click here to download an IBP screening tool.
