KOLOA, Hawaii — A successful lower eye blepharoplasty includes several treatment modalities and should be tailored to the individual patient’s needs, according to a speaker at Hawaiian Eye 2017.
“We need a way to customize our approach to take care of these different needs of patients,” Roger A. Dailey, MD, FACS, said.

“The best rejuvenation for most lower eyelids requires multiple types of treatment. Don’t try to hit a homerun with just one procedure or you’re going to get into problems and have unsuccessful results.”
Dailey categorized the treatment options into three general approaches: non-incisional, incisional and midface lift.
The non-incisional options include botulinum toxin, chemical peel, laser resurfacing and fillers. As for the incisional choices, Dailey said he does not perform a large number of fat transfers to lower lids because that can involve risk that can be bypassed with reliable fillers and other options.
When he does reposition fat from his choice of fat pad, whether temporal, nasal or central, he may opt to combine the procedure with CO2 laser resurfacing of the skin to alleviate degradation. Other options include a chemical peel and a pinch excision of the skin – by Joe Green
