Severity of visual field damage related to daily physical activity

Worse visual field damage from glaucoma correlated with less physical activity during typical waking hours and more fragmented episodes of activity compared with less severe damage.

“Physical inactivity is prevalent for visually impaired older adults,” E. Jian-Yu, MD, MPH, ScD(c), from Johns Hopkins Bloomberg School of Public Health, in Baltimore, Md., said during his ARVO presentation. “What matters is not only the amount, type and intensity of physical activity, but the fragmentation and diurnal patterns.”

The study involved 116 patients with normal to mild visual field damage, 96 patients with moderate damage and 25 with severe damage. Patients wore accelerometers for 1 week to estimate total active minutes per day and the number of activity bouts per day.

Patients with severe visual field damage had a higher fragmentation index and less active time per day (P < .05) but maintained a similar number of active bouts per day compared with the normal to mild group.

Similarly, every five-unit decrement in integrated visual field sensitivity correlated with an increased fragmented index and less active time per day (P < .05), but not in the number of bouts.

In time-of-day analyses, patients with severe visual field damage were less active compared with the normal to mild group throughout different periods.

“Although those with more advanced damage were less active overall, they did not frontload or backload their daily activity,” Jian-Yu said. “We acknowledge that further work is needed to establish whether these activity changes contribute to the disease severity or if they are downstream effects.”
