A well-designed case control study by Ye and colleagues provides answers to many important questions regarding reflux disease and adenocarcinoma. The authors reviewed Swedish patient databases from 1965 through 1997 to identify patients hospitalized with GERD-related diagnostic codes; they compared the risk of developing esophageal adeno-carcinoma in patients hospitalized with a diagnosis of GERD who were treated with antireflux surgery and in those who did not have operative intervention and hence were, it was assumed, treated medically.
