The primary goal of liver transplant immunosuppression is to prevent acute and chronic graft rejection that cause graft loss. Rejection is a host T-cell–mediated process. Foreign antigens in the graft are circulated to secondary lymphoid organs after reperfusion. Antigen presenting cells (APCs) process antigens and sensitize naïve T-cells via major histocompatibility complex–T-cell receptor (TCR) interaction.
