Anticipating higher-risk procedures should prepare the endoscopist for appropriate management and a game plan if a perforation actually occurs. High-risk procedures would include the following: If a high-risk situation is known, the scheduling of the procedure should not be at the end of the day or after hours, when radiological or surgical backup may not be readily available. Contacting the supportive services in advance to be on hand or to consult the patient in preparation for alternative management is prudent.
