Short stems implanted through a direct approach had higher revision rate in THA

Patients who underwent primary total hip arthroplasty with a short, tapered-wedge titanium stem inserted through the direct anterior approach experienced a significantly higher revision rate than patients implanted with other stem designs or approaches, according to results.

Researchers evaluated patients who underwent primary total hip arthroplasty (THA) from 2003 to 2009 through either a posterior or lateral approach using 232 extensively coated cobalt-chrome stems and 282 straight, dual-tapered, proximally porous-coated titanium stems. These were compared with 594 short, proximally coated, titanium tapered-wedge stems inserted through a direct anterior approach from 2009 to 2012.
